Does anyone know the correct distance from the ground to place player spawn points and hostage entities?

I normally just click on the ground in the 3D view when I have the info_player_terrorist or whatever, works fine for me.

It sometimes helps to place player starts a few units above the ground, as well, to ensure that players won't get stuck in the ground upon spawning in.
You might want to do the same with the hostages, to avoid the same problem.

While flying around in the 3d mode (click the 3d view and hit Z -- you can fly around with W, A, S, D). Now, while flying around if you're in entity mode you can click on any surface and create an entity there.

Select the entity tool (shift-e), then choose the spawn entity in the middle of the far left under objects (choose info_player_terrorist or info_player_counterterrorist), then in the 3d view click on the ground where you want that entity to be.
