Is is possible to use a "hot wobbly air" effect in hl2dm? In hl2 the engines of flying ships use it, and I want to put some lava in my map, and make it look truely hot.

Sure.

You want to use an env_steam set to the "Heat haze" type of particle. You can adjust size of particles (e.g. how large the warping effect is), length of the haze effect, et cetera. It may take a bit of tweaking to get it how you want it, but this is - AFAIK - the only way to do it.

Add an env_steam entity. Open the properties window (alt+enter). Change "Particle Type" to "Heatwave" to get the effect you want.

Make sure to play around with the starting size, ending size, distance, and direction of the heatwave. Play around till you get the result you like.

Also, for your lava map, you want the "Initial State" to be set to "On"

Thanks guys, might try it with a normal steam texture first to get the coverage right, then switch it over. Will probably make things easier to test.
