On map I have hostages that work fine. When I create the nav file they are scared of dropping into the sewers and going through doors. I tried the technique of compiling the map without any doors or sewer covers and then running the nav file. Then compiling the map again with doors back in and using the nav file.
This worked fine for the hostages to get through doors but they were still scared to drop into sewers. Also every round all players have 'Navigation Mesh created on a different version' written in white on their screen.
Any ideas how I can make the navigation mesh work with the doors in place?

I think that you should use nodes to get the hostages through doors.
Make ground nodes (info_node) on both sides of the door and create a
info_node_link to connect them. You might do something like that on the sewers too.

So would I place the link in the door and sewer cover?

Didn't work. They were still afraid to go through certain doors. Can someone please help me with this problem.
If you would like to check the .vmf file or if you think you can fix the problem on the .vmf file but can't explain how, please just ask and I will PM you the link to download it.
