A picture says more than a thousand word, so here my 2000-word explaination:

This is a very normal ceiling, world-brush and no compile errors (except for "3 degenerate faces" in vrad). I had this with a tools/blocklight texture somewhere else, where the invisible brush with the blocklight texture somehow intersected with the world. Moving the blocklight brush a little to the side fixed the problem.

So here, I thought that a near-by clip brush could have caused this, but it doesn't seem so. There is nothing that intersects, no tools brushes, no special geometry... I even tried deleting the brush and replacing it by a slightly different one, no luck. Whatever I place as a ceiling there gives me the missing triangle.

It's driving me nuts. Do you know anything like this, a possible solution?

Well you've tried destroy!rebuild! so that's out.

Does it happen at cordon compile? With a different texture? Is there a hidden visgroup at that location?

Failing that, I hate doing it but perhaps moving the entire map might help.

rather than rebuilding the brush exactly the same, what about slicing it in half or something. maybe the change would 'un-confuse' the compiler.

This is the weird thing: I did that! It didn't help at all.

I guess I'll check again for any hidden/non-obvious structures nearby next, thanks.

If you want to geto rig it, you could turn it into a displacement and place a nodraw brush behind it.

You could always place a second brush over the top of it. One unit lower.

or turn it in to a detail?

Sorry, i have no idea what could be causing it. Weird...

Awww... I don't want to go there... yet.

I'll try a recompile this evening, maybe some random rebuilding of the surrounding geometry will fix it.

Did you check alt-p check for problems >.<? You might have a degenerate face or a 'texture isn't perpendicular to axis' problem that can cause this weird type of thing.

It looks like you've been visited by the BSP hole fairy. The only way to fix it is to knock some teeth out and place them under your pillow... or maybe it was garlic... I forget. In any case good luck and I hope you have a good dentist.
